This folder contains two camera-ready Order Forms.

The HPFORM.EXE is an 81/2X11 form which your customers can fill out to place a 200-word homepage at our advertising website www.oldschooldjkennyray.com. When you get an order for $99, simply make a copy of the completed form and it to us along with $49 and we will typeset, program and post your customer's homepage for a period of one year. You make $50 profit for each order you get.

The CSTMFORM.EXE is a copy of the form your customers can use to order the customization of the Everything CD marketing showdisk. The price for customization is $139. When you receive an order, simply send us a copy of this completed form along with your wholesale payment of $89. We will produce the customized showdisk and send it directly to your customer. You make $50 profit for every order you receive.
